Kristen Brey

 

Kristen Brey is the Director of Population Health at UW Health.  She is responsible for the direction and leadership of programmatic, operational, financial, and personnel activities for the Office of Population Health.  The Office of Population Health at UW Health includes an Accountable Care Organization, Community Health Improvement, and Population Health Care Model.  The Population Health Care Model develops and provides programs for medically and socially complex patients anywhere they need care such as in clinics, hospitals, post-acute or home settings.  Kristen leads the development and execution of the population healthcare model strategy at UW Health.

Kristen brings work experience as a health system social worker, community relations, project management, program monitoring, and evaluation. Her interests include proactively addressing social determinants of health, systems change, and innovative partnerships to address complex issues.

Kristen earned her Bachelor of Science in Social Work from the University of Wisconsin- Whitewater.  She completed her Master of Social Work degree from the University of Wisconsin – Madison.  She holds an Advanced Practice Social Worker license in the State of Wisconsin.
